We’re looking for a Registered Nurse (RN) who’s ready to step into a leadership role supporting both residents and staff in a long-term care setting. As a Care Coordinator, you’ll oversee a dedicated neighborhood, guide your team, and ensure high-quality, person-centered care—while building strong relationships with residents, families, and colleagues.
 

What You Bring

• Active RN license in Wisconsin
• At least 1 year of nursing leadership experience in a long-term care facility
• Strong leadership and communication skills
• Ability to coach, develop, and support a team
• Passion for resident-centered care
