Commonheart Home Health and Hospice is a service of Illuminus–a not-for-profit organization and a faith-based community open for all, sponsored by the Western District of the Moravian Church, which has a tradition of caring since 1969.
Patients in need of home health and hospice services and their families are welcome, including people of all ages, faiths, and financial resources. We currently serve in southern Wisconsin in 32 counties.
